PROSTATE BIOPSIES

INDICATIONS:
Prostate cancer is the 2nd leading cause of death due to cancer in men. Since there are few prostate cancer symptoms, one way to help detect cancer early is through simple screening tests and biopsy.
SPECIMEN:
Prostate Biopsies
SUPPLIES:
1. Specimen Requisition
2. 10% neutral buffered formalin container(s) (of appropriate size) with formalin hazard label
3. Specimen Bag with a biohazard label
COLLECTION:
10% neutral buffered containers will be provided to clients for the collection of prostate biopsies. The containers of formalin for needle biopsies should be labeled as to site.
The client will need to label the container on the side with the Patient’s name and place in the appropriate transport container. The requisition should be folded and placed in the plastic pouch on the lid of the box. Complete test requisition including last and first name of patient, patient’s date of birth and social security number, body site and source of specimen collected. Label specimen container (using the labels provided on the requisition) with patient’s first name and last name, and body site/source. The container must have at least two (2) unique identifiers. Examples of unique identifiers: patient name, DOB, unique bar code, etc. Include pertinent clinical information, i.e., previous malignancy, radiation therapy, drugs, etc. Place container in a specimen bag with a biohazard label. Place the requisition in the side pocket of the specimen bag.
